Dr Purves will contact you to set up a convenient time for the online assessment. He will also send you the comprehensive assessment questionnaire by post. Please complete this and return it in the included envelope.

Once the comprehensive questionnaire is returned and the online assessment completed you will receive the report prepared by Dr Purves. 

Remember you also have the option of recording the assessment interview with Dr Purves
